Introducing People with Intellectual Disabilties to New Caregivers and Support Staff

The forms below are designed to help introduce individuals with intellectual disabilities or autism to new Doctors, Nurses, DSPs, or therapists. 

We recommend adapting these forms to best suit the needs of the individual, as needed and appropriate.

For example, a person with ID/A may have somebody new to them who needs to understand their abilities, behaviors, medical issues, how they eat/what they can swallow, their communication levels/skills and what they’re trying to communicate. We recommend the forms are accompanied by a picture of the individual, and that copies of these forms are entered into the medical records of the individual upon admission to a hospital as well as in in their primary residence and the providers of their daily activities or employment. The forms may also prove useful to EMTs or ambulance drivers.
